A nursing care plan defines the nursing care that a patient requires. What Is the Nursing Process All About?

The nursing process is the foundation stone to a nursing care plan. The first step of a nursing process is that the nurse requires collecting data of both types that is subjective as well as objective which is later required to be compelled into a systematic way like Marjory Gordon's functional health pattern. It further helps in diagnosing where the person requires nursing care. A nursing diagnosis is made on the basis of this data and the pattern created.

After checking a patient and making a nursing diagnosis, the next part is what sort of outcome should be expected from the diagnosis. In the diagnosis, the nurse also has to give a specific time by when a patient should recover. After the nursing diagnosis and time period for recovery is determined, the next is the nursing intervention to be provided. This nursing care plan needs to be followed to get the patient on the path to a speedy recovery. The care plan should have all the details and how often the plan has to be followed should be clear and easily readable by any nurse or medical attendee.

The time period (also known as the goal date) defined is the first step towards evaluation of a patient. There are many more factors such as if the patient met the specified time period for recovery or if he's not met it or whether the plan needs to be changed or some things require to be changed to improve the nursing care plan. In cases where the nursing care plan was defined for some small problem then the plan is stopped. If it is a chronic sickness, the plan is continued. And if it is problem or illness has not improved then the plan is changed or modified immediately to improve the plan. These plans are also given to various people at illness at various levels. There are plans for ?wellness? and ?high risk? plans.
