24Hrs Nursing & Clinical Care

The Home has a dedicated, trained and experienced team of more than 80 Care
Staff comprising Nursing Officers, Staff Nurses, Assistant Nurses, Nurse Aides
and Health Care Assistants to provide quality nursing care. They are responsible
for identifying, planning, developing, implementing and evaluating professional
standards of holistic nursing care practice in the Home.

Infection Control is a key factor to reduce morbidity and mortality among the residents
and staff. Education and strict compliance to infection control guidelines and
practices through in-house regular audits are a regular feature under the direct
supervision of the Director of Nursing.

Services provided by

    • Monitoring of vital signs (Blood Pressure, Temperature, Pulse Rate etc),
      Blood Sugar etc.
    • Assistance with Activities of Daily Living - Feeding, Bathing, Toileting,
      Dressing, Transferring and Mobility
    • Supervision of oral medication
    • Administration of intramuscular injections
    • Wound dressing
    • Insertion of nasogastric tube and nasogastric tube feeding
    • Change of urinary catheter
    • Tracheostomy Care

    • Health education for the residents & caregivers

Rehabilitation

Our team of physiotherapists, occupational therapists and speech therapists
target to maintain and / or improve the quality of life of the elderly through
restoration of strength, mobility, and independence in activities of daily
living. Enjoyable activities are also provided in a group context to reduce the
effects of institutionalization and prevent the elderly from the effects of
"learned helplessness". Services provided by Rehab team include:

    • Active / intensive Physiotherapy (PT) & Occupational Therapy (OT).
    • Maintenance Therapy.
    • Activity Groups (OT) - karaoke, opera, cooking sessions, arts & crafts
      etc.
    • Group Exercise Therapy (PT).
    • Chest PT.
    • Falls Prevention (PT).
    • Fitness Exercise (PT).
    • Wound Management (PT).
    • Recreational Activities.
    • Prescriptions of aids and appliances.
    • Home assessment and advice on home modification.
    • Reminiscence Therapy.

Medical Social Work

Medical Social Workers (MSW) are trained to provide a holistic approach to residents healthcare needs. They work closely with the Agency for Integrated Care (AIC) on the referrals from hospitals to the Home and also establish close liaison with other voluntary welfare organisations (VWO) to ensure high quality care for residents and their families. They attend to the social, environmental, financial and emotional needs of patients and families across the continuum of care offered by the various programmes and services eg.

Referral
Assessment of all Referrals for admission / rejection
Pre-admission counseling to ensure both resident and families to understand better the Home's background, its facilities and services, the various rules & regulations etc
The Application procedures for government subsidy
Means Testing

Discharge and Follow up services
Participate in the Discharge Care Plan of residents
Placement of Discharged residents in sheltered homes
Arrangement for Home Care services & special needs
Coordination of care plan after discharge to facilitate integration of the resident with his family and community

Psychosocial
Counseling for enhancing cognitive, emotional, interpersonal and/or social functioning
Increasing the resident's & family's understanding of an illness, treatment plan or grief
Follow-up on psychosocial adjustment of resident on discharge

Financial Assistance
Provide information on financial assistance & community resources e.g. IDAPE, Medifund, Eldershield, DPS etc
Assistance with payment of nursing home charges & short-term financial support to patients

Community Resource Education and Referral
Provide information & education on community resources & options available to meet identified needs of residents and families
Facilitate referrals to community agencies to ensure access to appropriate resources & continuity of care

Professional Medical Services

The Medical team comprises our Visiting Medical Officer, Visiting Consultant Geriatrician and Psycho-Geriatrician.The services provided include:

Provide comprehensive medical care for the residents.
Medical examination on admission.
Follow up at regular intervals.
Adhoc consultation.
Fortnightly Specialist consultations for selected residents and on an ad hoc basis.
Regular review and prescription of medications for all residents.
Identification of risk factors & formulation of appropriate treatment plans.
Liaison with the respective medical specialists when residents are hospitalized.
Ordering of necessary lab & other investigations.
Staff Training.
Health education for families.
Maximize rehabilitation potential of residents and facilitate transition from nursing home to community through teamwork and close liaison with all other Care Staff.

Day Rehab & Day Care Centre

The Day Rehabilitation & Day Care centre provides Physiotherapy; Occupational Therapy and Social Recreational activities for those who require follow up rehabilitation and social day care. Applicants can choose to have an hourly therapy program, a full day therapy and day care program or only day care program.

Day Rehabilitation Centre (DRC) provides Physiotherapy and Occupational Therapy services in the area of geriatrics, orthopedic and neurological rehabilitation such as

Parkinson's exercise management
Reminiscence therapy
ADL and IADL training
Vocational skills assessment
Cognitive behavioral therapy

Day Care Programme
Day Care Centre provides structured in-house activities for the elderly in a lively environment who would otherwise remain alone at home. Programmes and activities are designed to suit their functional abilities and fitness levels so as to maximize their social interaction and participation. Our Day care complement Day Rehab programme for those who require therapy services and supportive day care with the following programme and activities.

Group Therapy / Fun Fit Exercise.
Social Outings e.g. Visit to places of interest, shopping, community events
Social Recreational Activities e.g. Tai Chi, Games, Karaoke, Movie show, Art & Craft, Cooking and Gardening.
Maintenance Exercise Programme to maintain their physical fitness and daily living skills.


Additional Value-Added Services:
Consultation with Geriatrician & Psycho Geriatrician, Nursing Care and Health Screening are available on request at nominal charges.

Operating Hours:
Monday to Friday: 8.00am to 6.00pm excluding public holidays.

Charges:
Charges are based on subsidy status (MOH Means Test) and type of services which may be Day Rehab or Day Care or Day Rehab & Day Care. Daily Charges: $6 to $24; Monthly Package Charges: $75 to $600; Transport Charges: $3 to $12

Meals
Morning Coffee / Afternoon Tea: Snacks & Beverages
Lunch / Dinner: Nutritious lacto vegetarian meals

Transport
Adaptive transportation fitted with hydraulic lift and manned by therapy / healthcare assistant is available for DRC/DCC clients within Sembawang, Woodlands, Marsiling and Admiralty areas .We have stair crawl for those who are non ambulant and staying at non lift landing HDB apartments.

Application form can be downloaded from www.mft.org.sg or available at the Reception Counter of MFTNH -DRC/DCC. The form has to be completed and forwarded to the Manager, DRC/DCC along with the latest Medical Report of the applicant. Applications will be assessed and appointments made for an interview, within three working days of receipt of application form. If the applicant is found suitable, two working guarantors are required to sign the Standard Agreement Form.

Home Therapy
Home bound patients may have difficulty accessing community based therapy services at day rehabilitation centres and outpatient therapy services at hospitals due to their functional, enviourmental, adaptive transportation and escort service limitation.

Our trained and experienced Physiotherapists & Occupational Therapists can provide therapy services at your door step, in your home and also train the elderly to achieve independence in their Basic Activities of Daily Living (ADL). Caregivers (including maids) training is also provided so that they are able to continue with the basic care of their loved ones at home.
