Silicone implants never fully disappeared from the market, but for several years their use was limited and based only on specific medical criteria. Further, during those years any patient who received silicone implants was required to participate in a long-term study of health and safety.
The "silicone implant scare" seems to have been triggered by a combination of legal opportunism and some arguable medical "science", coupled with patient concerns over complications. After much debate, and based on extensive medical data, in November of 2006 silicone implants were once again approved by the FDA for use in breast enlargement surgery. A 13-member committee of the IOM (Institute of Medicine of the National Academy of Science) appointed by the U.S. Department of Health and Human Services determined the following at the end of their research:
"Evidence suggests diseases or conditions such as connective tissue diseases, cancer, neurological diseases or other systemic complaints or conditions are no more common in women with breast implants than in women without implants."
The FDA conclusion legitimized what most plastic surgeons already believed -- that silicone has always been a safe choice, and the decision about whether to use them should be left to the patient and his/her physician.
One of the restrictions included in the recent FDA approval is that the patient must be at least 22 years of age. Further, not all types of silicone were approved for general use. For instance, the newer, more cohesive implants - commonly known as "gummy bear" implants - require that the patient participate in a follow-up study. This doesn't mean the product is "unsafe" - it just means that the "gummy bear" implant wasn't one of the newer silicone products that made it into the November 2006 FDA approval. Manufacturers are currently working with the FDA to get these implants added to the list of approved gels.
There are significant improvements in the current gel implants. In the 1970's and 1980's silicone implants had a more liquid consistency, and the outside shells were thinner and more apt to leak. Today the implants have outer shells that consist of several layers and are made with an interior "cohesive" gel - one that is more stable and stronger. With the development of the newer gels there are also more implant "profile" choices for patients, helping to give them the look they want. The choice of multiple shapes and sizes are available in both saline and silicone implants.
Even though silicone implants once again have FDA approval, saline implants are still a good choice for many women. Saline is available for breast augmentation to women 18 years and older, unlike silicone where the minimum age is 22 years. Saline implants, like their gel counterparts, have silicone shells, but the inside is filled with salt water instead of the gel.
So what are the advantages and disadvantages of each? One of the advantages of silicone implants is that they feel more natural to the touch and there tends to be less "rippling" around the edges of the implant. For women who are thin or have small breasts or stretched skin, this is a distinct advantage. One disadvantage of silicone, however, is that the implants come pre-filled and a slightly longer incision is needed at surgery.
An advantage of saline implants is that they can be filled after insertion, making them ideal for those who prefer a transaxillary or transumbilical breast augmentation. (In the former, an incision is made in the armpit; in the latter, an incision is made in the navel and tunnels up to the breast. Both eliminate the possibility of any breast scarring.) Also, if a saline implant should leak it becomes readily apparent as the body quickly absorbs the saline. Saline implants also cost less than their silicone counterparts, and this is frequently a consideration.
When making the choice between silicone or saline implants, be assured that usually comparable and excellent results can be obtained with either type of implant. If you are over 22 years of age, the decision is entirely up to you. In the opinion of most plastic surgeons, either choice is a good one. The way to determine your own best choice is to meet with a board certified plastic surgeon to discuss the advantages and disadvantages of each.

Problems With Breast Implants
So, what cosmetic surgeries are really hurting specifically in these hard financial times, and more importantly, when will they pick up again? Also, is there a way you can use the rough economy to get a great price on a cosmetic procedure. You bet!
There is so much information and misinformation (doom and gloom media unfortunately) going on around our economy right now, that you'd think the US was on the brink of a collapse they will never come back from. While the situation is probably the most dire most of us will see in our lifetimes, we will come back from this, but it is going to take a lot of time. Unfortunately, most people in the know say it will take a few years, not a few months, to fully recuperate and get back to where we were for this to pass.
That doesn't mean our worlds have to stop, but it does mean for many of us that we need to make cutbacks on things that are not a necessity or things that could be put off without too much hassle of inconvenience. One of the items that falls in the "not a necessity" category is, you guessed it, one of the burgeoning markets over the past ten years, plastic surgery and cosmetic procedures.
According to most plastic surgeons, the past month or two have brought dramatic decreases in their business, especially when it comes to the more expensive procedures that range in the thousands of dollars category, such as breast augmentations and liposuction, both of which are costly because of the anesthesia and actual complicated surgery that is involved.
Conversely, other specialists that mostly do less expensive procedures such as facial fillers, Botox, and other more temporary, but less invasive procedures that do not involve surgery and thousands of dollars, may have actually seen a slight boost in their sales. In other words, lots of patients who may have otherwise been planning to get a facelift, mini facelift, or some other facial procedure requiring surgery may have opted instead for the less expensive, less invasive procedure to save money for the time being.
The quest for other alternatives that may be less expensive to these common and very popular surgeries has led many women to look for alternatives to breast implants, face lifts, tummy tucks, liposuction and the like. However, even those areas of opportunity or women and men have seen dips with the faltering economy.
They're finding that there are some great nonsurgical ways to look and feel fantastic and to improve their bodies as well as their self esteem, which is a great thing since surgery isn't for everybody and doesn't necessarily always accomplish what they patient wants.
Facelifts may be the way to go for some, but often times they do not have the desired outcome and look unnatural. I've seen a few women in the public eye who have gotten them and looked good, like Madonna and maybe Janice Dickinson would fit in that category, but for the most part facelifts just make the person look like they got a facelift.
There's no need for that any more with the abundance of cosmetic dermal fillers like juvederm and restylane, not to mention a bevy of at home products that can be used to blast away wrinkles and the fine lines and spots that go with aging. Such products I'm referring to are the at home soft light lasers, microdermabrasion systems, chemical peels that are available in the same strength as a dermatologist office (just use caution), and other home treatments that provide a world of difference when it comes to looking younger.
There are also ways to enhance the breasts without surgery, such as supplements that stimulate the growth and development of breast tissue, vacuum devices that stimulate growth, and a variety of creams and lotions, heck, even hypnosis for breast enhancement, that have worked for women seeking to enhance this part of their body.
When it comes to cellulite, areas where spot reduction is needed, and body contouring, there are a ton of body wraps that can be used at home, cellulite creams and lotions that work with massage to contour fatty areas, and even special shorts one can wear that reduce specific areas of the body through compression.
So you see, if you are one of the people who has put off that plastic surgery or that "dream procedure" you've been hoping for all these years, take heart that you can improve your appearance without surgery, and for a lot less money.
